The way it is configured to work is that Discourse will see the reply and flag it for moderator attention, then the moderators should move the reply to a new topic with a title.
That does not always happen and so sometimes replies get added that should not be there.
Without getting into specifics I am not entirely happy with the solution; it is a no win situation no matter what options are taken to take care of all the noted concerns. 
Here is what we see as admins when Discourse does flag a reply in the Announce category.
